Objectives. To review current literature concerning etiology and complications of life-threatening pathological childhood obesity. To evaluate the treatment results of two obese children who developed life-threatening complications of pathological obesity.

Methods. A overview of current literature. A retrospective analysis of the case history.

Results. As the medical literature shows, the number of pathologically obese children is increasing dramatically. This also increases the number of obesity-related threats to life-threatening complications which only adults had just encountered before. During the period of 2016-2018 two patients with pathological obesity and life-threatening complications admitted to the Children’s Hospital, at the Vilnius University Hospital Santaros Clinic intensive care unit. Complications were diagnosed in time and the treatment was successful.

Conclusions. The dramatic increase in the number of obese children leads to an increase in the number of complications associated with obesity, causing life-threatening complications. According to our retrospective analysis of two pathologically obese children treatment was successful, but transportation, examination and nursing was complicated due to the high degree of obesity. This was a significant challenge for the entire staff of the intensive care unit and the consulting team of doctors from various specialties.
