RENOVASCULAR HYPERTENSION DIAGNOSTICS AND TREATMENT. CLINICAL CASE AND LITERATURE REVIEW

Summary

Objective. The aim of the study is to review a clinical case of renovascular hypertension and compare it to the newest literature and guidelines.

Research methods. Newest European Society of Cardiology guidelines, PubMed database and Google Scholar search engine were used to conduct the research.

Results of the study. Arterial hypertension is a widespread disease. About 5–10% of the patients of this disease have a secondary hypertension. One of the most common causes of renovascular hypertension is renal artery stenosis. A gold standard test for this disease is digital subtraction angiography. However, other procedures are usually conducted first to confirm the diagnosis: duplex echocardiography, computed tomography angiography or magnetic resonance angiography. Although to treat this condition both medications and invasive procedures are commonly used, antihypertension medications is still the first-line treatment for the disease with angiotensin-converting enzyme inhibitors and angiotensin receptors blockers being the most important in a clinical setting. Invasive procedures like angioplasty with or without stenting are also used to treat the disease. Bypass surgery and removal of one kidney may also be considered, but these procedures are rarely used. After reviewing and comparing this clinical case with the newest literature, we conclude that the diagnostic and treatment applied to the patient at the Cardiology and Angiology Center of Vilnius University Hospital “Santaros Clinics” comply with international recommendations.
